Join the Registry on-line at any time. Or you can sign up when you obtain or renew your Ohio driver license or state I.D. card at an Ohio BMV by saying "YES" when asked if you want to be an organ donor.

You can also place your name in the Registry, or further refine your donation consent, by downloading and completing a Donor Registry Enrollment Form, and returning it by mail to:

Columbus, OH 43216-6784

This form may also be found in the Ohio Donor Registry Brochure available from your local Deputy Registrar.

If at any time, should you change your mind about donation, you can remove your name from the Registry using these same methods.
